“Everyone is absolutely amazing. Best place I've ever been to by far. Amazing all round Super satisfied”

Challenge
Here's what many clinicians know from experience: seeing the same provider over time changes outcomes. A systematic review in BMJ Open looked at 22 studies from nine countries. In 18 of them, greater continuity of care was linked to lower mortality. Three found no association, and one, based on insurance claims data, linked higher continuity to higher mortality.
Solution
KEL Healthcare Medical Services organizes care around the long view. Its board-certified physicians and nurse practitioners stay with patients across appointments, and every treatment plan is built around the patient's goals, routine and health history, then reviewed and adjusted as needs change.
Results
Thomas Nolan didn't describe a procedure or a diagnosis. He wrote that "everyone" was amazing and that he was "super satisfied." KEL's owner replied within two days, saying the team was thrilled to have exceeded his expectations and looked forward to welcoming him back.
Patients can leave as easily as they arrive

The evidence is observational, and the studies were too varied to combine into one effect size, so it doesn't prove cause. But the authors also pointed to other benefits tied to continuity: better communication, higher patient satisfaction, greater adherence to medical advice and less hospital use.
The challenge is that patients move on easily. In an AHA survey on provider loyalty, 70 percent of consumers who switched providers cited access as a deciding factor in choosing a new one. And 53 percent chose their new provider on a recommendation or referral from someone they trusted.

So a practice has two jobs. It has to be easy to reach, and it has to be good enough that patients recommend it.
A care model designed to be a long-term relationship
The services support that continuity. Comprehensive health assessments set a baseline. Chronic disease management brings regular monitoring and measurable goals for conditions like diabetes, hypertension, asthma, thyroid disease and high cholesterol. Preventive care and geriatric care carry patients through every stage of life. If a specialist is needed, referral services keep the primary care team involved.
Follow-up isn't an afterthought. KEL schedules visits to track progress and adjusts care as health improves and needs change, so each appointment builds on the last.
Access is built in too. Same-week primary care appointments are available at all four clinics, and walk-ins are welcome.
Results
That kind of response is how practices earn returning patients. Thomas's review is one of more than 486 behind KEL's 4.9 out of 5 Google rating, and KEL reports 98 percent patient satisfaction across more than 25,000 patients over 16-plus years.
A clinician's take on continuity
Every clinician knows what it's like to meet a patient for the first time. You spend the visit learning their history, their routines and their worries. The second visit, and the tenth, are different. You notice what's changed, you catch small shifts early, and the patient already trusts you enough to speak up.
That's what continuity offers. KEL's four-step journey, from scheduling to evaluation to a personalized plan to follow-up, is designed for exactly this, because the last step feeds the next visit. And satisfied patients are what make it possible. A team patients call "amazing" is a team patients will keep seeing, and that makes follow-up, monitoring and prevention far easier to do well.
What patients can take from this
If you're looking for a primary care home, ask whether you'll see the same clinicians over time and how soon you can be seen. KEL welcomes walk-ins and accepts most major plans, including Aetna, Cigna, UnitedHealthcare, Healthfirst, Fidelis Care, Medicare and Medicaid.
